This form is specifically used to request a check reissue.

Prime Therapeutics cannot change the address a check was mailed to unless the address has already been updated with NCPDP. For assistance with updating your address with NCPDP, or for claim processing and other general questions, please reach out to the pharmacy contact center at: 800.821.4795.

Prime Therapeutics requires that a check be outstanding for 30 days before a check inquiry is submitted.

To submit a check inquiry, please complete the form.  A replacement check will be issued to you in 2 to 4 weeks, provided the original check is still outstanding.

Please fill out the form in its entirety to ensure the issuance of a check; failure to do so can result in no issuance of a check.
