Overview
At Prime Therapeutics (Prime), committees made up of clinicians and other experts review medications to develop formularies that offer safe, high-quality and cost-effective products. Formulary determinations made through this review process are detailed in the quarterly formulary update notice. This notice summarizes additional formulary and UM changes.

Note: Members affected by tier increases, drug removals and/or select UM additions may receive written communication two months before the effective date.
